5. Consent to Automated Calls

By checking the applicable consent box on our forms and providing your telephone number, you expressly consent in writing to receive calls from or on behalf of Hear For You Hearing Aid Center at the number you provide, including calls placed using an automatic telephone dialing system, automated technology, and/or an artificial or prerecorded voice. These calls may include appointment reminders, follow-up care calls, and — where you have provided marketing consent — promotional calls.

Opting out of marketing communications does not affect appointment reminders or care-related communications you have consented to, and vice versa. Hear For You Hearing Aid Center safeguards health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Pennsylvania law. By opting in to receive communications from us, you acknowledge this risk and consent to receive appointment-related and care-related communications, which may reference your care at our practice, at the phone number or email address you provide. We limit the health information included in such communications to the minimum necessary. You may request that we communicate with you by alternative means or at an alternative location, and we will accommodate reasonable requests.

Uses and Disclosures of PHI

We use and disclose PHI only as permitted by HIPAA, including for treatment, payment, and healthcare operations. We do not use or disclose your PHI for marketing purposes without your written authorization, and we do not sell PHI.
