Personal information may be provided to subcontractors and service providers who perform support services on our behalf — such as customer service, appointment scheduling software, website hosting, and payment processing — and who are obligated to keep that information confidential and use it only to perform those services. We may also disclose information when required by law, or as permitted for treatment, payment, and healthcare operations under HIPAA (see Section 7).

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. 4. SMS/Text Messaging Program

When you provide your mobile phone number and opt in through our website forms, in person, or over the phone, you consent to receive text messages (SMS/MMS) from Hear For You Hearing Aid Center. Depending on the consent you provide, these messages may include:

  • Conversational and informational messages: appointment confirmations and reminders, follow-up care messages, hearing aid service and repair updates, and replies to your inquiries.

5. Consent to Automated Calls

By checking the applicable consent box on our forms and providing your telephone number, you expressly consent in writing to receive calls from or on behalf of Hear For You Hearing Aid Center at the number you provide, including calls placed using an automatic telephone dialing system, automated technology, and/or an artificial or prerecorded voice. These calls may include appointment reminders, follow-up care calls, and — where you have provided marketing consent — promotional calls.

Opting out of marketing communications does not affect appointment reminders or care-related communications you have consented to, and vice versa. Hear For You Hearing Aid Center safeguards health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Pennsylvania law. Health Information in Texts, Calls, and Emails

Please be aware that standard text messages, emails, and phone voicemails are not encrypted and could potentially be intercepted or viewed by others with access to your device or accounts. By opting in to receive communications from us, you acknowledge this risk and consent to receive appointment-related and care-related communications, which may reference your care at our practice, at the phone number or email address you provide. We limit the health information included in such communications to the minimum necessary. You may request that we communicate with you by alternative means or at an alternative location, and we will accommodate reasonable requests.

Uses and Disclosures of PHI

We use and disclose PHI only as permitted by HIPAA, including for treatment, payment, and healthcare operations. We do not use or disclose your PHI for marketing purposes without your written authorization, and we do not sell PHI.

We maintain administrative, technical, and physical safeguards designed to protect your information against unauthorized access, use, or disclosure. These include secure systems for storing patient records, access controls limiting information to staff who need it, and secure transmission of website form submissions.
